Address/Social Security Number Changes
Complete the change request form and submit it with proper documentation to the Admissions and Student Records Office.
Each student is responsible for informing the Admissions and Student Records Office of changes in name, phone, or address. Each applicant for admission is required to furnish a social security number. If you do not have a social security number a student number will be assigned through this office.
